Our Services

Suburban Primary Care provides a wide range of services in a convenient one-stop setting. Our Clinic welcomes patients during regular business hours and provides same day appointments whenever possible. Appointments can be made online or by calling our clinic.

COVID-19 Services For Our Patients:
COVID-19 Tests (tests are available only by appointment):
  • Rapid test (15 min) for those with symptoms.
  • PCR test (2-3 days) for those with known recent exposure or those with symptoms and a negative Rapid test.
  • Combined Rapid COVID-19 & Flu A / Flu B: for patients with symptoms or a viral infection.


COVID-19 Vaccine (please note that we will NOT be able to store the ultra-frozen vaccine):
  • Our practice has been approved by the CDC and IDPH to be a COVID vaccine administration site for our practice patients.
  • We will be able to store COVID-19 vaccines that require frozen (-5F to -13F) or usual refrigeration in our dedicated vaccine freezers.
  • This Website will be updated when the vaccine is more widely available and an email will be sent to all our web-enabled patients.

Adult Medicine:

Our physician focuses on prevention and early detection of serious diseases and provides healthy lifestyle management which includes cholesterol disorders, high blood pressure, smoking cessation programs, heart problems and weight loss issues.

Also offers the management of a wide range of complex, acute and chronic medical conditions such as bowel and intestinal problems, heart disease, obstructive pulmonary diseases.


Geriatric Care:

Our physician provides comprehensive medical care for older adults at various nursing home facilities that are minutes from our Clinic.

With the immediate availability of our physician and the on-site nurse practitioner, they are able to focus on frequent health problems affecting older patients, such as pain, falls, memory loss, incontinence and evaluate an elderly patient's ability to care for him or herself.


Telehealth Visits:

Telehealth visits are virtual office visits that let you video chat with a Suburban Primary Care physician from the comfort of your home or wherever you are with internet access! If you're out of town, stuck at work, or too sick to get out of bed, you can video chat with one of our physicians through your computer or your patient portal – the place where you already receive messages and lab results from your primary care physician. A physician can diagnose conditions and prescribe medicine from this visit with you.

There are many things you can speak with a physician about during a Telehealth visit. Some of these include, but not limited to

  • Colds and coughs
  • Pink eye
  • Follow-up visits
  • Sore throat
  • Congestion
  • Medication refills and checks
  • Urinary tract infections and more

Telehealth visits are covered by insurance, including Medicare and Medicare Advantage plans (please check with your insurance company). To video chat with one of our physicians, call Suburban Primary Care at 630-590-5751 to schedule your Telehealth visit. Make sure you have access to our Patient Portal on a video-capable computer or you will need to download the Healow app on your smartphone or tablet.

NOTE: There are some instances where your physician will ask you to come in, so make sure to discuss with our office staff what type of visit is best for you.

Our Clinic’s newest accomplishment is being certified as a Level 3 (the highest) Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). Diabetes Educational Classes:

Suburban Primary Care offers in-house diabetic educational classes with our clinician-educator, Natalie Salvino, FNP-BC.

Suburban Primary Care offers medically supervised Weight Management services that is clinically proven to provide safe, effective results.

The program is led by Suburban Primary Care’s Nurse Practitioner, Natalie Salvino, who will provide personalized, one-on-one evaluation and guidance to ensure patients are successful in leading a healthier lifestyle.

Annual Wellness/Preventive Exams:

Our physician performs yearly Annual Exams providing a preventive comprehensive plan of care for non-Medicare patients, ages 18 to 64 years old and Medicare beneficiaries, age 65 years and older, who have had Medicare part B coverage for at least 12 months.

Our non-Medicare patients will receive a preventive exam which includes History and Physical, as well as further physical and diagnostic testings.

Our Medicare patients will receive an initial Preventive Physical Examination, known as the "Welcome to Medicare" service. Once this service has been established, our physician and staff will maintain a personalized prevention plan annually where our Medicare beneficiaries will then be scheduled for subsequent Annual Wellness Visits (or AWV).

Our on-site laboratory is fully CLIA accredited, providing comprehensive clinical lab services to a wide range of age-specific patients. With a certified Phlebotomist on-site, patients are offered immediate lab services 5 days a week during the Clinic hours and rapid delivery of lab results.


Electronic Medical Records (EMR):

Our Clinic maintains all patient medical data in a paperless, electronic system designed to increase efficiency for our patients. All prescription orders, tests orders, reporting of test results and physician notes are computerized and stored safely within the system.

Patient Portal Access:

Our 24/7 secure and convenient web-based patient portal is designed to facilitate better communication with your physician's office.
